The Occupational Safety and Health Administration (OSHA) has announced its preliminary Top 10 most frequently cited workplace safety standards for fiscal year 2020. In an exclusive webinar with Safety+Health magazine on Feb. 26, Patrick Kapust, deputy director of OSHA's Directorate of Enforcement Programs, presented the preliminary data, and Kevin Druley, Safety+Health associate editor, moderated the session.
 
Although multiple standards swapped positions, the Top 10 violations from FY 2019 to FY 2020 did not change. Ladders (1926.1053) climbed to a top-five spot, and Respiratory Protection (1910.134) rose to the third rank from fifth. Additionally, the data show that Fall Protection – General Requirements (1926.501) is OSHA's most frequently cited standard for the 10th successive fiscal year.
 
"In a year that was defined by the ongoing pandemic, workplace safety became more important than ever," said Lorraine M. Martin, NSC president and CEO. "The OSHA Top 10 list reminds us why we must continue to focus on persistent safety risks as we navigate new challenges. These data help us pinpoint areas where we can improve so we can better prioritize workplace safety in the future world of work."
 
The Top 10 for FY 2020 are:
 
1. Fall Protection – General Requirements (1926.501): 5,424 violations
 
2. Hazard Communication (1910.1200): 3,199
 
3. Respiratory Protection (1910.134): 2,649
 
4. Scaffolding (1926.451): 2,538
 
5. Ladders (1926.1053): 2,129
 
6. Lockout/Tagout (1910.147): 2,065
 
7. Powered Industrial Trucks (1910.178): 1,932
 
8. Fall Protection – Training Requirements (1926.503): 1,621
 
9. Personal Protective and Life Saving Equipment – Eye and Face Protection (1926.102): 1,369
 
10. Machine Guarding (1910.212): 1,313
 

If you have credentials to train operators of scissor lifts or aerial boomlifts issued before June 2019, you must complete the MEWP Trainer Update Program and test, accessible free of charge through the IVES Member Dashboard.
 
If you attend a Recertification program, your documentation and renewed credentials will not be processed until you complete the online Update program.

Requalification Materials

Requalification Materials are available for counterbalanced forklifts, narrow aisle forklifts, powered pallet trucks, rough terrain forklifts, aerial boomlifts, and scissor lifts . Requalification programs must be delivered at regular intervals as mandated by government regulations for forklift operators. Re-training and re-evaluation of forklift and mobile elevating work platform (MEWP) operators must also be performed based on incidents attributable to operator error or on the observations of the employer.

Case report: #17NY014

Date of incident: May 13, 2017
 
A 53-year-old truck driver was struck and killed when fence pipes fell from a flatbed trailer during unloading at a fence sales company. It was raining when the driver arrived, and the company owner was the only person onsite. The trailer carried 18 bundles of pipes arranged in three tiers, with six bundles in each tier. The owner started to unload the pipes with a telehandler while the delivery driver stood on the other side of the trailer winding securement straps. As the owner lifted the fifth bundle on the top tier, the sixth bundle slid and fell to the ground, trapping the delivery driver underneath. The owner immediately drove the telehandler to the driver, lifted the 1,400-pound bundle off him and called 911. Emergency medical services arrived and transported the driver to a hospital, where he died of crushing injuries three hours later.
 
To help prevent similar incidents:
 
  • Employers should develop and implement standard operating procedures for loading and unloading.
     
  • Employers should implement measures to safely handle cargo that is slippery and slick, or can easily roll, shift or fall from trailers or material handling equipment.
     
  • Freight companies should develop and implement standard operating procedures for drivers to follow during loading or unloading at customer sites.
     
  • Freight companies should provide loading and unloading safety training, as well as ensure drivers understand and follow procedures.

Question:
 
Hey guys. I can't find anywhere where it says certified operators have to carry a wallet ID card. Yes, we have them to give out, but when someone loses the wallet ID, they are told not to run the machine. All I see in OSHA regulation is that we have to retain records and proof of training. The wallet ID is just a convenience. Is this correct?
 
Answer:
 
Yes, you are correct in that the card is not required by OSHA. I know of some companies that do not even give them out. However, I have also been told that if operators can produce their cards should an OSHA inspector request it, things seem to go a little easier. Thanks for checking in with us!
